Recommendation for PRTF.
17-year-old Trans Male patient who presented to PrairieCare following a mental health crisis assessment due to auditory and visual hallucinations, self-harming behaviors , refusal of medications, and refusal to eat. Patient was recently administratively discharged from RTC.
Risk Factor: Self harm
This term refers to instances of isolated, non-repetitive aggressive behavior, such as a single incident of physical aggression or verbal aggression.

Pt has a hx of MDD, GAD, PTSD, RAD, insomnia and unspecified disruptive, impulse control and conduct disorder. Pt has previous inpt MH admission from 12/18-1/23/25 at UMMC. Per review of the record pt has recently been at Bar None and Divine group home. Pt has hx of foster care placement and has been under legal custody of Hennepin county. Pt has a hx of abuse and neglect. Mother has mental health concerns and CHIPS order indicated hx of civil commitment. Pt was in her father’s care and removed due to concerns for abuse. Maternal uncle died by suicide. Pt had multiple recent ED visits. Pt has been with her grandfather for a few days after running from her last placement and being found on the streets.

14-year-old female who admitted to PrairieCare Inpatient Hospital due to increase in suicidal ideation. She has a history of depression, anxiety, PTSD, Autism, and multiple suicide attempts. This is her third psychiatric inpatient hospitalization, has engaged in day treatment, PHP, and outpatient psychiatric services.

Due to poly trauma patient has had lengthy hospitalization with multiple orthopedic injuries. Her psychiatric needs surpass inpatient physical medicine and rehab needs.
In need of psychiatric crisis respite facility with 24/7 supervision and/or psychiatric residential placement. Has not yet been screened through Anoka County QRTF screening team.

She was released from Prarie Care 3 days ago with a recommendation from her psychiatrist there to be admitted to a residential living facility. She was released as a final attempt of living independently with outpatient care. patient reports cutting her thigh with a piece of glass and she told her mother she did not feel safe going to school , came to ED for help.

Presented from partial hospitalization program (Clara’s House) for worsening aggression and self-harm. Has had in-home skills worker and outpatient providers. Was referred to Clara’s House for worsening non suicidal self-injurious behaviors. Recommended residential treatment due to attempting all lower levels of care.

12-year-old male presenting to PrairieCare Inpatient Hospital due to an altercation with his mother after not wanting to go to school. Following this altercation, his mother found a suicide note. He is a history of depression, anxiety, PTSD, mood/dysregulation resulting in intrusive SI. This is his 2nd psychiatric inpatient hospitalization. He has engaged in PHP and outpatient psychiatric services.

15-year-old female presenting to PrairieCare Inpatient Hospital as a step up from Newport RTC due to an increase in intrusive SI. She will not be returning to Newport per parent request due to SA and elopement while in programming. She has a history of depression, anxiety, ADHD, and PTSD. This is her 4th psychiatric inpatient hospitalization. She has also engaged in PHP and outpatient psychiatric services.
