Patient presented to the ER via law enforcement for aggressive behaviors at shelter care resulting in property destruction. Patient is not able to return to the shelter and does not have any safe disposition options.

Pt came to ED 3/25 after being on the run for a few weeks. Was previously at hotel crisis respite prior to being on the run, damaged property there and can not return. County is current guardian, though patient turns 18 very shortly and will be his own guardian at that time. History of behaviors, verbal and physical aggression and property destruction. Has been denied from shelters due to behaviors and previously denied from RTCs in and out of state, though due to age would no longer be an option. Refuses all mental health services and has not had a DA since 2020. Is on CADI waiver. Recommendation on weekly call to discuss benefits of IRTS program/treatment with him.

Patient reportedly having increased aggressive episodes at home towards parents and property destruction. Parents unwilling to have patient in the home at this time. Working with family and case manager on shelter or alternative placement need.

Trying to get a Case manager assigned. Anoka non secure 45-day assessment program could include a psycho-sexual assessment (? age requirements?).
Patient presented to the ED by mother’s boyfriend due to concerns of sexual abuse. History of physical and sexual abuse and recently has been perpetrating on his younger siblings and having increased behavioral concerns and aggression. Placed on a welfare hold for placement, court ordered to out of home placement. Has been denied by 7 foster homes, no shelters accepting patients his age. CPS currently has custody.

Patient presented from Prairie Care PHP due to aggression and dysregulated behaviors. PHP unwilling to accept patient back, mother unwilling to take home at this time and cites need for RTC. No RTC referrals made currently. Patient needs support in short term placement while outpatient team supports long term plan and placement. Has been in the ED 4 other times. Back and forth from ED to home. No letter of rec. for residential at the moment. Working on it. There is a case mgr, working on a care conference currently.

Patient brought in to ED by case manager after her group home discharged her due to ongoing behaviors and aggression. Case manager did not have alternate placement and therefore patient now boarding in ED until new placement can be found. Patient in need of shelter or crisis respite placement while longer term placement is found. Awaiting funding.

Comes from shelter placement (Von Wald) due to aggression. Has probation with court on 3/14/24. Voluntarily placed with Rice County by family due to safety concerns. The CASII score is 6 with strong recommendation for wilderness residential programming, as he has been through a few ‘traditional’ residential programs, so he has some buy-in for wilderness type residential. Looking for shelter vs group home as waiting for residential programming. Parents hesitant to look at out-of-state programs.

Brought in my EMS after being kicked out of the Bridge for Youth shelter- tripped over 4-foot fence and broke right ankle while eloping. Has been staying with friends. Grandmother is guardian and lives in North Carolina. Returned to MN in Foster Care in December of 2023. Has a case manager through Brown County, Denise Kamm (507-276-3411). Denise reports patient has run away from multiple group homes, foster homes, and shelters. Denise the CM saying she has exhausted resources, places, and shelters for this patient. Denise says trying to find a shelter but likely not going to be able.
Currently in the HCMC Emergency Department but is pending admission up to our pediatric floor for boarding until placement can be determined. Please see above contact information for follow up. Has a broken ankle that is splinted and in a stirrup brace, she will have crutches on discharge. No pertinent medical history noted. High elopement risk.
