This pt discharged from our ED on 8/6 after being here since 7/22, went to a Beacon crisis home for approx 24 hours and returned to the ED again on 8/7, after aggressive behavior. Previous number was MHF-M12-4418. Also boarded in our ED in April 2024 for 20 days, and at different hospital in June. Hennepin County has temp custody and he is on DD waiver. Currently looking at a CRS home that requires exception bed approval through DHS, Grafton, remains on the MCCP crisis pool list, and judge is making determination if returning to Mom may be an option. Has been to Zane crisis, Beacon crisis, VOA Bar None, and done PHP with FV.

Pt came to our ED on 8/10 from Grandparents home who were providing foster care and there was an altercation. Grandparents are refusing to take pt back. Hennepin County CPS/ICWA have temp custody. Pt not appropriate for IP due to fecal incontinence and struggles with communication. County is looking for DD waiver CRS and doing a MNchoices assessment on 8/16.

Pt came to our ED on 8/9 from parents home after an altercation. Parents are refusing to pick pt up. They report pt is involved in some gang activity and verbal and physical aggression toward them at home. Has been to 3 residential programs, and multiple placements Northwoods, Gerard and Bar None. He continues to exhibit behaviors in all settings. County does not feel another residential would be beneficial as the last 3 haven’t improved behaviors. Therapist is currently working on IQ testing but unsure if that can be completed in the hospital. Team is looking at starting MNchoices process to get waiver options.

Patient came to our ED on 7/22 after an altercation with mother and mother is refusing to pick pt up. Was previously in ED on 7/12 for same reason. Was reportedly discharged from Nexus East Bethel 7/12. Has been denied by Northwoods, Gerard. Was somewhat successful in the past with day treatment, CM is exploring day treatment options.

Under Hennepin County custody. Came to the hospital after assaulting a peer and staff at crisis respite. Has been in multiple placements (residential, group homes, foster care and shelters) and has been terminated from them due to aggression. There were concerns for psychosis prior to admission but symptoms have stabilized with medication.

Pt arrived in our ED on 5/23. She had previously been at Children’s Hospital (inpatient) for 40 days, went to a crisis placement for around 24 hours when she started to aggress physically and was brought to our ED. Crisis respite refuses to accept pt back. Has diagnosis of autism and lower IQ, unsure of the number. Was a recent DA completed by Nystrom that team is waiting on to submit referrals for residential. Mom refuses out of state options at this time. Has name in crisis bed pool via MCCP, on DD waiver.

Pt came into our ED on 6/13 after a verbal altercation at home with mom, where she left the house and walked to the police station who called crisis and brought pt to ED. Mom is refusing to pick up. Its reported pt was in residential treatment until April 2024, and prior to that was in PHP through Fairview. Mom reports that Case Manager is looking into residential treatment again however we have been unable to reach the CM and their offices are closed the rest of this week. Unsure if there is a recommendation for residential or county approval for that LOC.

Discharging today 6/20 to group home
Came to our ED solely due to not having another placement. Has boarded in our ED in the past. Was previously at Bar None Shelter plus where she assaulted others and was taken to JDC. Was in JDC for a week when a judge ordered her to be removed, so CPS brought her to the ED as they had no other place to go. CPS is searching for group home placement as that is what the County’s juvenile screening team deemed the appropriate level of care.

Update: Discharging today 6/20 to hotel crisis respite
Came to the ED this time on 6/9, after refusing to do chores and punching a wall and tree. Dad is refusing to pick up. Was previously in our ED 5/4-5/18 and 5/25-5/28, returned home with Dad both of those times. Case Manager is looking for waiver paid placement, and has been for over a month. Had interview with potential provider 6/12- they are potentially going to do a variance for him to live in adult home if accepted (he’s 3 months from 18). At baseline has some auditory hallucinations, ongoing aggression- primarily verbal, occasionally property, along with some suicidal ideation.

Update: Discharged to Aspen House but was having suicidal ideation so returned to ED 5/25. County working with Prairie Care Residential for potential placement.
Pt came to ED on 5/10 via foster care parent due to suicidal ideation. Foster parent refusing to take back due to these behaviors. Was previously in North Homes residential and was in inpatient with us from 2/5-2/13. County reports they are exploring residential and PRTF, waiting for DA to come back with recommendation. Completing referral for Nexus YCT. Pt’s mental health is being negatively impacted by boarding, she has been continuously sobbing in our ED about being here.
