Allina- Mental Health Complex Care Coordinators

Mental Health Complex Care Coordinators (MHCCC) provides care coordination for Allina Health patients who are established in our outpatient Hospital Based Clinics (HBC) patients who are under the care of our psychology and psychiatry providers. MHCCCs assist patients in navigating services within and outside of Allina Health to facilitate mental health stabilization.
