Youth came to our ED 4/24 from a foster home that is refusing to let her return. Reported she has recently boarded in multiple other hospitals. County is currently seeking group home placement, in both therapeutic group home settings and CRS and crisis respite settings via the waiver. The last DA done earlier this month has a recommendation for OP services which the team does not agree with so they are also seeking an assessment program to determine appropriate level of care, North Homes denied, waiting to hear on Gerards assessment program.

Youth came to our ED on 4/9 after being on the run for 3 weeks. Has a long history of elopement and some aggression as well. Is open with CPS, ACT team, and DD waiver, and there is a VPA for placement currently. County team is looking at all placement options- all waiver options, RTC, out of state. Has been denied many places already primarily due to the elopement behaviors.

Youth came to our ED 1/9 after running away from family member she was placed with, unable to return to that family. SEY concerns. Has been denied from multiple SEY/shelter/crisis options due to behaviors. County is working on placing with Aunt in WI but needing ICPC to be finalized first. Were hoping that would be completed last week, however home study for that occurred this morning 1/22. County reports she is not eligible for waiver.

Youth came into our ED on 12/28 after altercation at hotel crisis respite. Was in this setting for approx 5 months, and prior to that was boarding in our ED from 5/28-7/3. Hotel crisis will not take her back. She has 4 different felony charges pending, awaiting a rule 20 assessment. Most placements including non secure corrections settings will not accept due to aggression. Team is working toward a single site CRS home, but that is a few weeks from being completed, waiting on provider’s waiver enrollment with DHS and finalized staffing. Last DA done a few weeks ago recommended GH with supports, and scored a level 5, however not pursuing treatment placement as all MN options have denied and pt was in RTC in Florida for 2 years previously.

Youth came to ED 9/22 from Foster Home after aggressive behavior and is unable to return there. Was recently in FV IP unit until 9/19. County has temp custody and is looking for other foster options, but he has been circulated on the list since end of July with no acceptance. Expedited MNchoices is reportedly being requested.

17-year-old female who presents to PrairieCare Inpatient Hospital due to suicide attempt with increased SI and SIB.
This is her 2nd psychiatric hospitalization. She has engaged in PHP and outpatient psychiatric services.
For RTC – she will need funding through Ramsey County.

Was in metro area with adoptive parents. Placed at Youth Shelter in St. Cloud. Came to hospital. Shelter d/c’d her and refused to pick her up. Ramsey probation and SW already involved. Adoptive parents want to relinquish any rights to patient as aggression is focused on mom.

Youth came to our ED on 6/13 straight from a Missouri RTC that he was discharged from due to aggressive behavior. Came to us as there was no other place for him to go. Has CMH and they are reporting they are not seeking another placement at this time and recommend pt return to adoptive Mom, or we contact CPS. Mom also requested we contact CPS so she can get further assistance and potentially out of home placement. No waiver at this time.

This youth has boarded with us 4x prior. She was most recently boarding with us last week 5/28 and a day later physically aggressed on 4 of our ED nurses and was brought to JDC. She was discharged from JDC due to a previous rule 20, brought to another ED who discharged her immediately and returned to us again, only due to lack of placement. County is working on renewing her MNchoices, searching for waiver placement, out of state and is seeing if management would approve a hotel crisis respite.

Youth came to our ED on 5/24/25 after physical altercation at Red River Valley placement, and they refuse to let her return. Has history of physical aggression and a rule 20. Has boarded with us multiple times prior- 12/5/23-12/29/23, 2/13/25-2/27/25, and 3/6/25-3/12/25. Looking both in and out of state for placements that might take her.
