Mom transferred to rights to dad 2 years ago through a CHIPS petition. McLeod county CPS has been working voluntarily with the family for awhile (CPS worker Daisy Vinkemeier 320-300-8724). Dad had been doing well up until recently when he relapsed. Dad was admitted to the hospital yesterday at the same time as patient due to medical issues. Due to this, McLeod County has placed patient on a health and welfare hold 8/28 until the team can get to court to take temporary custody of the patient. At this time there are not any kinship options for the pt.

Came to our ED from her CRS home, who issued a temp suspension and are not allowing her back at this time. On DD waiver with DD CM. No other CM involvement currently. Has IQ of 60. Last DA completed in March recommended PHP. On list for crisis respite.

Recommendation is for level 6 RTC/PRTF – patient is currently inpatient.
15-year-old Female from RTC level 5. Patient admitted to inpatient after struggling with self-regulation after recent inpatient discharge. Patient has had 5 inpatient hopsitalizations since March 2024 with attempts at individual therapy, PHP, RTC (elopment), and In alignment with CMHCM, seeking RTC level of care. Referrals have been sent to Nexus East Bethel, Bar None Haven
