Update: 8/31/23 Approved for Gerard admission week of 9/18, pending county funding. Also accepted at NorthHomes, 3-6 month waitlist.

Patient was brought in by medics after she was physically aggressive with a staff member at The Aspen House, where she has been living for two months.
Patient has adoptive parents and adoptive siblings. She was adopted when she was two years old, and has recently reconnected with bio-dad.
Patient had prior diagnoses of depression, ADHD combined type, anxiety, neurocognitive disorder related to maternal meth abuse while patient was in utero, and learning disability. She’s been in M Health FV IOP, since she’s been there. She does not participate in groups or other activities much. She’s been in an inpatient mental health unit, mostly at M Health FV, at least four times. The last admit was 4/21-4/27/2022 at FV. She’s had the following RTC placements: Eau Claire Academy, 4-8/2022, CRTC 8/2022-1/2023, and Avanti 1-4/2023. She receives Psychiatry and Therapy services through the group home.
She has had two prior suicide attempts by choking herself.
UPDATE 8/24/23 – remains at Abbott Northwestern.
ASD, lower functioning, home bound schooling, aggression towards family and staff. On wait list for MCCP crisis bed, out of state referrals being looked at. As of 4/11/23, still waiting on placement. got the waiver for group home, but no providers available.
Patient is an 11-year-old male presenting to Children’s Emergency Department due to increasing behavioral escalations at home. Patient has a history of trauma. Patient has not had stable housing and has been in several different placements within this last year.
Patient does not meet inpatient mental health criteria due to no ongoing or current suicidal ideation, thoughts of self-harm, or thoughts to harm others and was recommended to discharge home to his foster home with his grandmother.
Pt is presented to the ED by the police after running away from home approximately two weeks ago. Pt had gotten into an argument with her mother and no longer felt safe there. Pt reports being physically abused at her mother’s house. Pt had been staying at a man’s house since running away from home, has been having unprotected sexual intercourse with him, and has a history of being sexually exploited for money. Pt is also on probation for unknown reasons.
