Teens in Transition (TNT) is the oldest program at North Homes Children and Family Services.  TNT provides a community, group setting support for male-identifying youth between ages 15 and 20. The program focuses on serving young people who have experienced or face a heightened risk of sexual exploitation.  Participants without identified risk factors may also access services.

Support focuses on developing the skills required for independent living. Participants engage in trauma-informed, youth-driven care shaped by individual strengths and goals. Core components include:

  • Daily skill-building groups centered on independent living
  • One-on-one coaching
  • Recreation and leisure planning
  • Household management practice
  • Vocational skills groups and exploration
  • Community-based outings
  • Workforce development and opportunity to gain employment in the community

Residents may access services through local providers, including mental health counseling, medication management, substance use support, advocacy following sexual harm, spiritual and cultural resources, routine medical and dental care, and off-site education through ISD 318.

Participants leave the program with increased capacity to manage daily responsibilities and navigate adult systems.
