Sleepy Eye Medical Center

We are a small Critical Access Hospital in Sleepy Eye, I am a Care Coordinator/Discharge Planner in the hospital. We do not have a Social worker so my role requires me to fill some of the gaps in this area. I also assist with placements from the ER with mental health and chemical dependency placements.
